Oaklawn, St. Joseph County (Madison Center) is a State of IN PATH Provider. PATH stands for Projects for Assistance in Transition from Homelessness. PATH program funds service people with serious mental illness experiencing homelessness.

PATH providers are local public or nonprofit organizations that have receive US Government PATH funding to provide services to people who are not currently connected to mainstream services.

Some Services PATH Programs provide, but are not completely inclusive for all of the programs (Some PATH providers offer all of these services, while others focus on providing one or two of them.):
Outreach Services
Screening and Diagnostic Treatment
Habilitation and Rehabilitation
Community Mental Health
Substance Abuse Treatment
Referrals for primary health care, job training, housing, educational assistance
Housing services as specified in Section 522(b)(10)

    I'm in need of housing. I'm on the South Bend Housing list and have been number 20 all year. I'm not getting any answer or anywhere with them. I've been to the center for homeless and as great as a place as I think it is I can't go there because I have a dog and I'm not giving up or giving away my dog. I need him for support. I was going to Oakland and doing what I was suppose to. I don't know what or why but I kept getting forgotten about or blown off when it came to getting help with housing. I'm on ssi and sadly I can't do this on my own.

    I just left center for the homeless a couple weeks ago I've always dealt with my mental illness alone can you give me any information as to where I start getting help
